Non-communicable diseases (NCDs) are a public health challenge that requires a promotive and preventive approach based on community participation. The Islamic Health Movement program was implemented in Medokan Ayu Village, Rungkut District, Surabaya, to increase public understanding of a healthy lifestyle that is integrated with Islamic values. This activity aims to increase health literacy based on halal and thayyib principles and identify risk factors for NCDs through health screening. The implementation methods include health education, measurement of understanding through questionnaires before and after education, as well as cholesterol and blood sugar level checks. The results of the evaluation of 70 participants showed an increase in average understanding from 55.7% to 90.6%. The screening produced 72 examination data on participants aged 17–73 years. As many as 51.4% of participants had cholesterol levels of ≥200 mg/dL, while 27.8% showed blood sugar test results of ≥200 mg/dL. These findings confirm the need for follow-up through clinical confirmation, nutritional counseling, increased physical activity, and periodic health monitoring. The Islamic Health Movement program has the potential to become a model of community empowerment that integrates health education, sharia values, and early detection of NCD risk factors. The sustainability of the program requires cross-sectoral collaboration to encourage sustainable changes in healthy living behaviors.
